3/1/2024 EnchantaVaughn Approved QMHPA greeted Chevelle upon arrival to the home setting. Upon arrival to the home setting, Chevelle appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her afternoon. Chevelle verbalized to QMHPA that her afternoon was going well. Chevelle did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Chevelle current thoughts, feelings and emotions. QMHPA discussed with Chevelle about her medication management. QMHPA engaged Chevelle in a discussion about any upcoming appointments that she have. QMHPA engaged Chevelle in a skills building activity. QMHPA explained to Chevelle that the purpose of this activity was to assist her with creating a calendar of appointments with location, date, time and transportation needs. QMHPA questioned Chevelle on her level of depressiveness between 1 through 10. QMHPA explained to Chevelle about allowing things to cause her to become depress and using proper calming skills. QMHPA assisted Chevelle with being able to identifying triggers that cause her to experience depression. QMHPA demonstrated with Chevelle appropriate ways to express her feelings through role play. QMHPA explained to Chevelle being calm, listening to what others have to say, take a minute to think, then come up with a positive solution. QMHPA engaged Chevelle in a second skills building activity. QMHPA explained to Chevelle that the purpose of this activity was to assist Chevelle with positive vocabulary terms to assist her with improving her communication skill and expressing her feelings in an appropriate behavior. Chevelle appeared to be receptive to QMHPAâs interventions and services that were provided to her during the session. Chevelle informed QMHPA that she is adhering to her medication but as she mentioned to QMHPA during the last session it is not working fast enough for her. Chevelle expressed to QMHPA that she knows that she has to wait until it get into her system she just has to be patient. Chevelle said she needed to create a calendar and have a calendar in her wallet. Chevelle said she was feeling good and was at a 0 level of depression. Chevelle said she was depressed the night before but she went to bed. Chevelle said she was having dreams about fires and did not know what that meant and she had a slight panic attack worrying about her apartment catching on fire in her dream. Chevelle said she couldn't sleep well. Chevelle was cooperative with expressing her feelings but she stated if someone talk to her or come off at her wrong she just lose it. Chevelle said it's hard for her to think before talking at times especially when she is mad. Chevelle said she going to tell it like it is. Chevelle would be calm for a minute but she is able to identify her depressive and anxiety type symptoms.
